Advanced Care Home Health is seeking a compassionate and highly motivated Registered Nurse Case Manager to join our dedicated team. As a Registered Nurse Clinical Supervisor, you will play a crucial role in providing exceptional care and support to our staff and patients in the comfort of their own homes. Responsibilities:
- Provide oversight of all patient care services and personnel. Oversight to include the following: making or assisting the scheduler with patient and personnel assignments, assuring that patient needs are continually assessed and assuring the development, implementation and updates of the plan of care are including all physicians involved in the plan of care.
- Conduct comprehensive assessments of patients' health status and individualized needs to develop personalized care plans.
- Coordinate and manage patient care in collaboration with physicians, therapists, and other healthcare professionals.
- Provide skilled nursing care, including medication administration, wound care, and monitoring of vital signs.
- Monitor patients' progress and update care plans accordingly, ensuring that interventions align with their evolving needs.
- Educate patients and their families on disease management, self-care techniques, and available community resources.
- Advocate for patients' rights and ensure their preferences and goals are integrated into the care plan.
- Maintain accurate and up-to-date documentation of patient assessments, care provided, and outcomes achieved.
- Collaborate with interdisciplinary teams to facilitate seamless transitions of care and improve overall patient well-being.
- Adhere to regulatory guidelines, company policies, and industry best practices.
Qualifications:
- Valid Registered Nurse (RN) license in Oregon.
- Minimum 2 years of nursing experience, preferably in home health or a related field.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals.
- Ability to prioritize and manage multiple cases efficiently.
- Demonstrated compassion, empathy, and sensitivity in providing patient-centered care.
- Knowledge of Medicare guidelines and home health regulations is desirable.
- Proficiency in electronic health record (EHR) systems and basic computer skills.
