We are looking for a Clinical Behavioral Specialist to provide direct service to our community members in need of behavioral health support, case management support and independent living skills. Our Clinical Behavioral Specialist:
- Provides outreach behavioral health assistance and support to ensure participant’s successful transition from institutional setting to the community;
- Determines participant’s s needs by establishing a safe trusting relationship with participant, families and caregivers.
- Creates behavioral support plans; provides the necessary training and guidance to community members.
- Provides specialized cognitive counseling; provides training as needed to carry out successful transition to living in the community.
- Maintains continuity of care by communicating changes in participant’s condition and circumstances to all care team members.
- Coordinates with local housing authorities and other housing resources to support participant in current or future placement.
- Supports participant during physical move from institutional setting to community setting,
- Performs other duties as assigned.
Requirements include:
- A Master’s Degree or higher in Social Work, Psychology, or related field, or the equivalent combination of education and experience. Associate level clinicians are encouraged to apply.
- Must have access to private reliable means of transportation.
- Ability to effectively use computer and communication technology, including Microsoft Office Suite: Excel, Word, PowerPoint, OneNote, Teams.
- Ability to maintain professional boundaries and practice self-care.
Physical Requirements of the job:
- Must be able to lift 25lbs
