Description:
Under the direction of the Program Manager, the TAY Housing Navigator will provide housing location, navigation, and stabilization services to youth exiting the County of Los Angeles Child Welfare System (18-24 years old) who have been identified and referred by the Department of Children and Family Services (DCFS). The Housing Navigator will be responsible for assessing needs and stabilizing youth exiting care to housing that is appropriate for their needs. Housing may include, but is not limited to, transitional housing, time-limited subsidies, shared housing, general affordable housing, and permanent supportive housing. The Housing Navigator will serve as the primary support liaison for youth transitioning out-of-care and serve as the primary contact with referring partners from the Department of Children and Family Services.
The TAY Housing Navigator will provide individualized and culturally competent support, create a housing stability plan, accompany youth to housing appointments, and support youth throughout the entire housing search, placement, and move-in process. The Housing Navigator will provide direct supportive services to youth, as needed, to remove any barriers to maintaining stability housing and advancing their career and education goals. The Housing Navigator will interface with local landlords to identify available housing as well as property managers at housing sites. After stabilizing youth, the Housing Navigator will assist them in developing an individualized career plan, with a focus on maintaining housing stability, transitioning to longer-term or permanent supportive housing options, and accessing sustainable wages through workforce development programing and training. The Housing Navigator will serve as a direct liaison to CRCD’s Workforce Development Department to provide relevant and quality career pathways training and employment opportunities to youth.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Ensure each youth completes an initial intake and needs assessment.
- Assist youth in determining personal goals and plans to achieve housing stability and career pathway goals through the development of an Individual Action Plan (IAP).
- Assist youth in accessing services that address the needs and goals of their IAP, including but not limited to mental/physical health services, mainstream benefits, and education.
- Provide housing stabilization services to youth, which may include topics like money management, life skills, accessing transportation, understanding their lease, self-advocacy and other topics based on the needs of youth.
- Partner on and participate in outreach efforts to youth experiencing homelessness in South Los Angeles.
- Establish and cultivate relationships with landlords and other housing providers to build a portfolio of housing options for youth.
- Participate in care coordination meetings with the Department of Children and Family Services and other relevant agencies.
- Enter data in required databases, compile and submit reports that track client status and support services provided.
- Compile and submit monthly service reports to supervisor that track client status and the supportive services which have been provided to them.
- Attend all required partner meetings and relevant community meetings and staff development training as assigned.
- Data collection/entry into HMIS & Salesforce database to assemble reports and evaluate program outcomes.
- Maintain client case file for accuracy of all eligibility required documents as per CRCD case filing protocols and procedures.
- Conduct a minimum of one face-to-face contact with each client on a weekly basis.
- Act as a liaison among the Project Manager, the property management company and any other relevant parties to mitigate and issues that could impact housing stability.
- Act as a liaison and coordinate linkage to services with numerous public social service agencies and community providers, including CRCD’s Worforce Development Department.
- Provide youth with move-in support, eviction prevention services and supportive services to support stability and career development.

Requirements:
- 1-2 years’ experience or related work and a Bachelors-level Degree; or an Associates-level Degree and 5 or more years’ experience providing housing- based support services to young people facing multiple barriers to housing stability.
- Experience working with youth experiencing homelessness and housing instability, and/or have had experience in foster care, probation, mental health systems.
- Reliable, flexible team player who works well with minimal supervision, who has a good work ethic, and an ability to set and maintain personal boundaries.
- Demonstrated ability to work on multiple projects simultaneously, and to meet deadlines.
- Knowledge of South Los Angeles community & social services resources.
- Background in Mental Health services, Trauma Informed Care, and/or Harm Reduction a plus.
- Reliable transportation: valid driver's license, and car insurance as required by law, as appropriate.
- Proficient in Microsoft Office Suite.
SALARY RANGE
$45,000-$55,000 annually
