The Field Nurse Practitioner reports to the VP of Clinical Operations, with accountability for providing clinical judgment, organization, and evidenced-based analysis in keeping with the highest standards of our model of home care.

Primary Responsibilities
The Field Nurse Practitioner shall have following duties:
Provide general medical care to various and/or specific patient levels: adults, geriatrics, and pediatrics.
Perform comprehensive evaluations including history and physical exams for risk adjustment and other regulatory assessments.
Address both chronic and acute primary care complaints, and be able to ascertain medical urgency.
Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom; able to work within an environment of limited resources and therefore use diagnostic tests judiciously and appropriately
Create and implement a medical plan of care.
Provides care coordination and preventative care services for geographical as well as virtual targets who are stratified by risk.
Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization.
Perform face-to-face in-person visits in a variety of settings.
Additionally, perform face-to-face synchronous video communications using the Tele-Health platform based on business needs and leadership direction.
Prescribe medications as appropriate.
Perform timely documentation in medical records in an electronic medical record computer system.
Engage in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws.
Collaboration: The Field Nurse Practitioner will work closely with other clinical and business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the clinical objectives of CINQ Care Inc.’s Care @ Home model of care.
Knowledge: The Field Nurse Practitioner will provide clinical subject matter expertise, seek specialty consultation as appropriate, and is responsible for knowing when a patient needs physician oversight and when the patient's medical condition may be beyond the scope of knowledge of the NP.

Qualifications
The Field Nurse Practitioner should have the following qualifications:
Education:
Certified and registered as a nurse practitioner (NP) by the State Education Department to practice as an NP in IL.
Currently registered and licensed as a registered professional nurse (RN) in IL.
Graduated from a nurse practitioner education program acceptable to State NP Nursing Board or be certified as a nurse practitioner by a national certifying organization acceptable to State Nursing
Board. Board certification: AANP or ANCC REQUIRED
Completed pharmacology coursework acceptable to State Nursing Board
Applied for nurse practitioner certification with State Nursing Board
Experience:
Previous experience as Nurse Practitioner, 5 years or more
Previous experience in Health Home programs as a licensed clinician, especially in the management of chronic conditions
Experience with underserved populations facing socioeconomic barriers to health care.
Fluency in a language in addition to English is a plus.
Immunization and phlebotomy skills

PHYSICAL DEMANDS
Requires sitting and standing associated with a normal office environment. Manual dexterity needed using a computer keyboard. While performing the duties of this position, the employee is regularly required to talk or listen.
The employee needs to be able to see and frequently is required to use hands or fingers, handle, or feel objects. The employee is frequently required to stand, walk, sit; and reach with hands and arms. Ability to travel frequently by car and/or public transportation.
Work Environment:
Will require regular travel to patients’ homes, physician offices, hospitals, sub-acute facilities, community partners, and non-home-based market offices.
Location: Hybrid: Mainly a Field position and will make visits to patient homes, hospitals, facilities, CinqCare practices, CinqCare main offices, other entities as appropriate.
Salary: $120,000.00 - $140,000.00 per year
