Position Summary:
We are seeking a passionate individual to provide case management services to homeless families referred to Family Promise of Greater Orlando for the Youth Homelessness Demonstration Program (YHDP). As a YHDP Success Coach you will be responsible for managing a caseload of youth with multiple barriers, as they work towards stabilized housing, promote sustainability, and prevent returning to homelessness. This position will provide support to youth 18 to 24 years old, with the focus of assisting them obtain housing, connect to community resources and meeting identified and unidentified needs. The candidate must be comfortable conducting home-visits, have flexibility in their work schedule to meet varying of their clients, a strong sense of professionalism and the ability to multitask.
This is a full-time non-exempt position reporting to the Director of Program Operations.
Highlighted Duties and Responsibilities
· Work with youth to develop housing stability plans that incorporate goals for both the youth and children, related to core goal areas: finding stable housing, increasing income, and family stabilization and support.
· Work with youth to find appropriate program that would best fit participant and or children, Rapid Rehousing, Permanent Support Housing, Transitional Housing, etc.
· Develop and implement parenting and life skills workshops and programs aimed at improving parenting skills, child development knowledge, and family dynamics, life, and daily living skills.
· Assist youth who are experiencing homelessness or at imminent risk of experiencing homelessness to navigate resources and connect to educational and or employment opportunities.
· Provide client-centered, trauma-informed case management services active and solutions-focused approach to case management
· Analyze client barriers and challenges to employment or education, as well as strengths and skills.
· Create appropriate and valuable education and/or employment plans for youth based on individualized goals and needs. Assesses clients’ vocational and/or academic functioning on an ongoing basis utilizing background information and work experiences.
· Support client job readiness skills including but not limited to resume and cover letters, interviewing, time management, and professionalism. Conducts job development and job search activities directed towards positions that are individualized to the interests and uniqueness of the youth.
· Meet as needed with youth on caseload to develop case plans, review progress, coordinate, and broker services, and assist clients in meeting the established goals.
· In collaboration with families and the Housing Location team, assist with housing search and placement process, sometimes accompanying youth to view units, advocating for the youth with landlords and assisting the youth with applications and other paperwork.
· Network with community resources for housing, education, employment, substance abuse treatment, mental health treatment, training, etc. and makes referrals as appropriate.
· Complete required assessments with families.
· Obtain signed release of information forms and communicate regularly with staff at treatment programs, and vocational programs.
· Enter the client information in the Homeless Management Information System (HMIS) and submit documentation of all clients related interactions on a timely basis.
· Understand specific need of youth who identify as LGBTQ+. Effectively and respectfully serve LGBTQ+ youth.
· Assist with duties at the Day Center as needed.
Qualifications
· Bachelor’s degree in social work (or related field) required. MSW preferred.
· Minimum of two years’ experience in direct service/administration in the non-profit sector related to housing, shelter, or family service required.
· Knowledge of evidence based best practices
· Experience collaborating and partnering with other social service providers.
· Experience with HMIS or similar data base is a plus
· Ability to think critically and utilize good judgement.
This position requires a high level of professionalism, attention to detail, and the ability to work independently.
Job Type: Full-time
Pay: $17.00 - $20.00 per hour
