Charlin Health Services job is to utilizing resources to care for you while continuing to care for patients throughout the metroplex.
We are looking for a Registered Nurse to join our Home Health team. The ideal candidate will have experience in acute care, outpatient, long term care, rehabilitation center, and/or assisted living facility settings. The successful candidate will be responsible for providing quality care to patients in their homes, assessing patient needs and developing care plans, administering medications and treatments, and providing education to patients and their families.
Responsibilities:
- Coordinates patient/client care. Coordinates and integrates care provided by Agency staff, including contractual providers.
- Ensures communication with all physicians involved in a patient’s/client’s plan of care and other healthcare practitioners.
- Assess the needs of patients and families in the home health setting.
- Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.
- Coordinates referrals.
- Participates in activities relevant to professional services including development of qualifications. Recommends resources needed to provide care. Makes patient/client and personnel assignments.
- Assures that patient/client needs are continually assessed. Assures reassessment of the patient’s/client’s needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient’s/client’s condition, at the physician’s request, and after hospital discharge.
- Provide direct patient care in the home health setting.
- Develop and implement individualized patient care plans.
- Educate patients and families on disease processes, medications, treatments, and other healthcare related topics.
- Monitors and participates in the QAPI program and assures appropriate corrective measures are performed.
- Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient/client care activities to prevent contamination and transmission of disease.
- Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
- Participates in the Agency sponsored in-service trainings.
- Collaborates on development and implementation of policies and procedures which guide and support the provision of care. Collaborates with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient/client care.
- Monitor patient progress and modify care plans as needed.
- Communicate with other healthcare professionals to coordinate patient care.
- Provide emotional support to patients and families.
- Document patient information in accordance with regulatory requirements.
Job Type: Full-time
Pay: $82,000.00 - $95,000.00 per year
