The New York and/or New Jersey Licensed Professional Counselor collaborates with fellow treatment team members to ensure the provision of quality care tailored to the unique needs and care levels of each patient within an outpatient treatment setting.
They are responsible for delivering direct services to patients and their families enrolled in PAX Health, including individual, group, and family therapy sessions.
This position involves direct work with Workers Compensation patients.
Responsibilities:
- Receive, review, and promptly respond to intake referrals upon assignment of the case.
- Conduct Bio/Psych/Social assessments, diagnostic impression, and formulate treatment plans under the guidance of a qualified Clinical Supervisor.
- Meet for individual sessions with patients at a frequency determined by the ongoing assessment of progress in treatment.
- Complete and assist clients in aftercare and discharge planning
- Provide direct clinical services, including individual, group, family and/or couples therapy to
- clients and their families under the guidance of a qualified Clinical Supervisor.
- Provide case management services as needed, including referrals to outside resources when necessary
- Ensure cohesion of all services documented within the clinical chart (including but not limited to services provided by case management, medical providers and the laboratory), and assists with compliance of various standards.
- Maintain accurate, thoroughly documented client records with a quality of documentation that meets all applicable standards in the Electronic Health Record (EHR).
- Utilize EHR to input all clinical documentation.
- Attend and participate in treatment team meetings as scheduled to ensure quality of care for clients
- Attend staff development and general staff meetings
- Attend staff and clinical trainings
- Communicate with Medical Director and other prescribers to ensure quality clinical services are provided to the client on a timely and as needed basis
- Participate in and attend supervision/consultation with the Clinical Director.
- Collaborate with members of the multidisciplinary treatment team to ensure effective delivery of a full spectrum of clinical services.
- Comply with local, state and federal confidentiality laws and regulations
- Collaborate with professionals both within and outside the agency, as appropriate.
- Demonstrate sensitivity and responsiveness to cultural differences.
- Abide by PAX Health code of ethics and model values.
Qualifications:
- Must be able to communicate with patients and staff members through various forms of communication, including email, phone.
- Computer literacy is required. Must be able to use Word, Excel, etc.
- Experience on CHARM is a plus.
- Masters in Counseling, Psychology or related field AND:
- Current license in New York and/or New Jersey to practice independently as LPC
