Position Description & Primary Job Duties:
Work collaboratively with the Eisner Health Primary Care Provider (PCP) as an integral part of the interdisciplinary team the Clinic RN is responsible for providing nursing care to patients including support with clinic overflow, triage, checking vital signs, perform POC testing, manage and administer medications, and support with wound care and any other clinical needs in accordance with RN scope of practice. The Clinic Nurse (RN) will support the clinic staff by managing ongoing care, complex clinical situations, assisting with procedures as needed (within RN scope of practice) and provide clinical support to ensure efficient operations of Eisner Health.
- Performs direct nursing patient care including patient assessments, and other nursing clinical duties that require assessment.
- Effective Triage of patients walking into the clinic or seeking same day care via the call center. Act as the point person for triage; focusing on maximizing Eisner Health’s internal ability for ER/Urgent care diversion though walk in clinic management and clinical escalations.
- Perform and document ordered procedures and care in clinic, including medication management and administration within RN scope of practice, as directed by PCP.
- Remain alert to pertinent input from other team members, participants, and caregivers and update the clinical team promptly of any changes in patient condition or medical status.
- Optimizes patient flow throughout the clinic by promptly addressing any issues that may disrupt the patient care process, and effectively communicates with clinicians regarding factors that may impact the delivery of timely and quality care, in accordance with industry standards.
- Ensures strict adherence to universal precautions as established by the Center for Disease Control and Prevention, Occupational Safety and Health Administration, DHS/OA and clinic standards.
- In collaboration with the Quality Improvement team, participate in quality improvement activities to identify areas for improvement and implement strategies to improve patient outcomes.
- Understands own and team members scope of practice and escalates issues as appropriate.
- Ensures patient safety related but not limited to, medications and procedures utilizing the five rights.
- Dispense ordered medications to participants while in the clinic and educate and/or provide as-needed teaching to participants, family and caregivers regarding the disease process, self-care techniques, and prevention strategies, as assigned.
- Maintains and protects patient confidentiality.
- Assist with medical emergencies as defined by license by scope of practice.
- Assist with refilling, clarifying and triaging any urgent medications, if the PCP or ordering provider is not urgently available.
- Review and sign off on any pre-procedural documents if the PCP or ordering provider is not urgently available.
- Answer phone calls from internal or outside facilities and /or patients needing to urgently communicate with a provider.
- Utilize Standing orders for routine testing such as, but not limited to, lead and hemoglobin testing, predefined medications such as albuterol, acetaminophen, ibuprofen.
- Assist with medication reconciliation for transition of care patients.
- Communicate basic lab results such a lead, hemoglobin, COVID results, and simple emergency follow up calls to patients
- Assist and oversee competencies/training of staff for existing and new medical assistants, LVN’s.
- Maintains and enforces a working knowledge of facility infection control procedures.
- Administer, observe, and train medical assistants, licensed vocational nurses and other relevant staff on the administration of Immunization’s, injectable and oral medications.
- Provide clinical support, within RN scope of practice to Medical Assistants performing procedures in clinic as needed.
- Ensures emergency (code) equipment is kept up to date.
- Under the direction of the clinic supervisors, prioritizes, delegates, and supervises work assignments appropriately to ensure completion of patient care activities.
- Assist with interpretation of employee/occupational PPD testing.
Qualifications:
- FQHC experience preferred
- Pediatrics and Adult Medicine experience preferred
- Must be a graduate of an accredited school of nursing w/a valid, current CA Registered Nurse license to practice.
- One-year of clinical experience as a Registered Nurse preferred.
- Ability to communicate in English and Spanish
- Prior administrative and supervisory experience in an outpatient family practice, internal medicine, pediatrics clinic is preferred. Working knowledge of direct outpatient care and management methods and practices in a community clinic setting or similar health care facility and manage clinic nursing services to achieve the delivery of efficient, quality patient care services.
- Basic knowledge of local, state and federal regulations relating to care of patients in a clinic setting; and prior experience with county, state and federal programs such as CHDP, FPACT, EWC, Medi-Cal, Managed Care, and Title X is preferred.
- Knowledge of common safety hazards and precautions sufficient to establish a safe work environment.
- Ability provide direction and serve as a resource to others; complete required continuing education programs; work proficiently in emergency and/or stressful situations; maintain confidentiality of patient/client information; be compassionate and comfortable working with pediatric patients, and effectively maintain a positive working relationship with the medical staff, public, patients, communicate effectively, both orally and in writing
- Knowledge of medical procedures and conditions and ability to understand, follow and successfully perform duties, including blood collection in accordance with administrative and clinical policies, regulations and procedures.
- Ability to develop and maintain cooperative relationships with staff members, patients and clinicians and work as a member of a multi-disciplinary team.
- Handles high volume of patients, internal/external customers, and appropriately manages frequent changes, delay or unexpected events
- Experience with electronic medical records, preferred.
- Covid-19 vaccination and booster required as per California State Law
- Valid Basic Life Support (CPR)
