Job Overview:
We are seeking a Physical Therapist to join our nonprofit partner in their PACE program. The ideal candidate will provide specialized, preventative medical care to elderly patients, focusing on promoting healthy aging and managing age-related health conditions within the home.
Duties:
- Conducts an initial Physical Therapy assessment
- Develop personalized care plans tailored to each patient's needs and goals
- Establishes and instructs participants and caregivers in safety recommendations, adaptations, exercises, and therapeutic home exercise programs as indicated.
- Provides oversight and supervision of the Physical Therapist Assistants (PTAs) and
- Rehabilitation Aides in accordance with the Illinois Physical Therapy Act.
- Collaborate with interdisciplinary teams to ensure holistic care for geriatric patients
Qualifications:
- Degree in Physical Therapy
- 2+ years of working experience, working in senior care
- Bilingual (English/Spanish) highly preferred
Job Type: Full-time
Pay: $95,000.00 - $120,000.00 per year
