Overview:
The Delegating Nurse Case Manager (RN CM/DN) for NextGen Health Services Inc is responsible for assessing the nursing care needs of people served/supported by the agency. The RN CM/DN will visit the residential group homes / people supported at least every 45 days, or more frequently if necessary, and provide a written review to the agency. The RN CM/DN will create comprehensive care plans, train staff on nursing care plans, assess staff proficiency with medication administration and person-centered care, review medical records, and communicate with departments.
The RN CM/DN would be responsible for maintaining status as a DDA RN CM/DN by attending two annual DDA Nurse meetings and becoming proficient in clinical review of the (HRST) Health Risk Screening Tool. The RN CM/DN will serve on the intake committee, which will evaluate potential persons seeking admission into any of NextGen's Support Services.
Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and assessing patient conditions
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans
- Perform various medical procedures, such as ostomy care, CT scans, ventilator management, and botulinum toxin treatment
- Assist with airway management and emergency response situations
- Educate patients and their families on disease management, treatment plans, and preventive care measures
- Coordinate with other healthcare providers to ensure continuity of care
- Document patient information accurately and maintain medical records
- Adhere to all safety protocols and infection control measures
Experience:
- Minimum of 2 years of experience as a Registered Nurse in a clinical setting
- Experience in primary care, senior care, or specialized areas such as mammography or DME is preferred
- Strong knowledge of medical procedures and protocols
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Proficient in using electronic medical record systems
