Job Summary:
We are seeking a skilled and detail-oriented MDS Coordinator to join our healthcare team. The MDS Coordinator will be responsible for coordinating and overseeing the completion of Minimum Data Set (MDS) assessments for our patients. This role requires strong medical documentation skills, knowledge of physiology and anatomy, and familiarity with internal medicine and behavioral health.
Duties:
- Coordinate and complete MDS assessments accurately and in a timely manner
- Ensure compliance with state and federal regulations, as well as facility policies and procedures
- Collaborate with interdisciplinary team members to gather necessary information for assessments
- Review medical records, conduct interviews, and perform physical assessments to gather data
- Analyze data collected to determine appropriate coding for MDS assessments
- Maintain accurate and up-to-date documentation of patient assessments
- Communicate assessment findings to appropriate team members
- Participate in care planning meetings to discuss patient needs and goals
- Provide education and training to staff regarding MDS documentation requirements
Experience:
- Previous experience as an MDS Coordinator or similar role preferred
- Strong knowledge of medical terminology, physiology, anatomy, and standards of care
- Familiarity with internal medicine, behavioral health, ophthalmology, and related specialties
- Proficiency in using electronic health record systems such as Epic or Athenahealth
Requirements:
- Valid certification as an MDS Coordinator (preferred)
- Bachelor's degree in nursing or related field (preferred)
- Excellent attention to detail and organizational skills
- Strong communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- Knowledge of Medicare/Medicaid regulations related to MDS assessments
