Details:
As a Triage Nurse, you will be responsible for assisting patients’ needs over the phone, providing medical advice within your scope of pratice, coordinating care, and determining the urgency of patients’ situations to ensure they receive appropriate care. You will play a crucial role in helping patients and their families navigate through difficult times by offering support and guidance.

Delivers telephonic nurse assessment, advice and instruction to patients and caregivers after normal operating hours within their scope of practice

Provides the case manager and management team with information regarding changes in the patient’s condition after telephonic contact
- Provides health care instruction to the patient/caregiver in a clear and concise manner as appropriate per assessment and plan of care

Teach the patient and family/caregiver self-care techniques as appropriate
-
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required

Assign nursing visits accordingly
Qualifications:
- Current LVN or RN licensure (in good standing) in the state(s) of practice with one year of experience under license

Current CPR Certification

Previous hospice experience preferred

Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.

Work closely with patients on a 1-1 level but get strong support from a patient care team.

Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.

Ability to learn an electronic medical records system.
