We have immediate openings for Home Health Registered Nurses (RNs) Case Managers to provide skilled nursing services to patients at home. This role initiates the nursing plan of care with preventative and rehabilitative nursing procedures.
Only Full-Time hours available. We have multiple territories throughout our 60 mile coverage area in the Willamette Valley. Each RN sees approx. 5-7 patients a day with an ongoing caseload of approximately 25-30 patients.
Responsibilities:
- Make initial evaluations and regularly reevaluates the patient's nursing needs including updating the plan of care.
- Provides skilled nursing services to include but not limited to: catheter care, wound care, disease management teaching, and medication management.
- Works closely with the patient/family/caregivers and other members of the interdisciplinary team
- Supervise, delegate, teach and train aspects of patient care to other members of the interdisciplinary team
- Ability to meet the productivity expectations of the agency.
Job Requirements:
- Current & valid license to practice in Oregon
- Current & valid OR Driver's license
- Current & valid car insurance
- CPR certified
- Dependable car
- Ability to function as a member of the Interdisciplinary team communicating at least weekly with the team
- Excellent critical thinking skills, time management, & ability to be flexible
- Case management
- 1 year of acute experience preferred but not required
- Home health or SNF experience preferred but not required
- Must be able to complete and pass health screenings.
- Must be able to pass a criminal background check.
