ENHANCED CARE MANAGEMENT PROGRAM SUPERVISOR

Enhanced Care Management provides a whole-person approach to care that addresses high-need Medi-Cal members’ clinical and non-clinical needs.
ECM Program Supervisor is a fulltime position that provides clinical leadership and program management.
Responsibilities/Essential Functions
▪ Provide supervision and oversee the daily operations of the Enhanced Care Management program within the Care Management Department, including staffing, scheduling, and coordination services delivered.
▪ Monitor and evaluate the performance of the ECM program, using data-driven approaches to identify areas for improvement and implement necessary changes.
▪ Supervise, support, and evaluate the performance of ECM Case Managers & Community Outreach Workers, providing guidance, training, and mentorship to ensure optimal client outcomes and service delivery.
▪ Manage new client intake; oversee and streamline processes with referral sources.
▪ Supervise outreach procedures to ensure that the department meets monthly, quarterly, and annual enrollment targets. ▪ Establish and maintain strong relationships with community partners and stakeholders, promoting the services offered by the ECM Program and Sourcewise, this includes advocating for the needs of clients with complex healthcare conditions.
▪ Monitor implementation of Care Plans; Recommend revisions or updates as necessary to accomplish the client’s goals.
▪ Provide clinical oversight of Care Manager assessments and interventions. Monitor quality assurance, by ongoing review of electronic records and client charts.
▪ Monitor caseloads to ensure client safety and program compliance.
▪ Act as liaison between ECM staff and Managed Health Plans to ensure compliance with program regulations.

▪ Responsible for coordination, review, and accuracy of all ECM claims are entered timely for processing monthly.
▪ Lead implementation of policy changes department wide in partnership with Director of Care Management.
▪ Contribute to the planning and facilitation of staff meetings, team activities, and staff training.
▪ Coordinate with all Sourcewise departments (Care Management, Operations, IT, Finance, etc.) to implement ECM program goals and objectives.
▪ Crisis intervention, as needed.
▪ Supervision of social work student interns, as needed.
Education and Experience
▪ Current RN or LCSW license
▪ Minimum of two years of experience with program supervision
