Job Summary:
We are seeking a dedicated Director of Case Management to oversee and lead our dynamic team in providing exceptional care coordination services. The ideal candidate will have a background in nursing, experience in ICU or Level I Trauma Centers, and a strong understanding of case management principles. As the Director of Case Management, you will be responsible for ensuring efficient documentation review, implementing behavioral therapy strategies, coordinating discharge planning, and collaborating with home health providers. Your expertise in mental health counseling and adherence to industry standards will be crucial in optimizing patient outcomes.
Responsibilities:
- Lead the case management team in developing and implementing comprehensive care plans for patients.
- Conduct thorough documentation reviews to ensure accuracy and compliance with regulatory requirements.
- Utilize behavioral therapy techniques to support patients in achieving their healthcare goals.
- Coordinate discharge planning processes to facilitate smooth transitions for patients from hospital to home or other care settings.
- Collaborate with home health agencies to ensure continuity of care for patients post-discharge.
- Provide mental health counseling and support to patients and their families as needed.
- Maintain up-to-date knowledge of industry standards and best practices in case management.
Skills:
- Proven experience as a nurse, preferably in an ICU or Level I Trauma Center.
- Strong background in case management principles and practices.
- Proficient in documentation review and maintaining accurate records.
- Knowledge of behavioral therapy techniques for patient support.
- Expertise in discharge planning and post-discharge care coordination.
- Ability to collaborate effectively with home health providers.

Pay: $150,000.00 - $165,000.00 per year
