Scope of Work:
The Mental Health Consultant (MHC) will be responsible in providing direct patient services to the ARP HVP NFP clients and their families who are experiencing difficulties and complex psycho-social issues, as well as mental health interventions as it relates to maternal and child health and early childhood development as part of the home visiting program.
Duties will include but are not limited to:
a. Provide in-home mental health services to clients who are enrolled in ARP HVP NFPaccording to their assigned Service Planning Areas (SPAs)
b. Provide home visiting and participating families with assessments and expert mentalhealth or developmental consultation, guidance, evaluation and/or referrals. The mentalhealth screenings and therapy may utilize Outcomes Questionnaire 45.2 (OQ.45.2),Youth Outcomes Questionnaire (Y-OQ 2.01 ), Cognitive Behavioral Therapy and othermental health questionnaires, based on assessments and needs.
c. Reach and maintain an active caseload of up to 25 ARP HVP NFP families.
d. Provide ongoing consultations to the ARP HVP NFP team which consists of nurses,supervising and administrative staff to fully integrate maternal mental health and earlyand latent child mental health practices into home visiting.
e. Incorporate clinical and programmatic best practices on linking clients to appropriatemental health/developmental/behavioral health care, and engaging clients and familiesin mental health service systems.
f. Establish detailed and coordinated treatment plans with clients and the client's supportsystem and other health professionals to set up goals within three (3) working days ofthe service provided.
g. Integrate proper mental health charting, mental health assessment, and evaluationmeasurement practices into home visiting.
h. Adhere to licensed clinical social worker documentation, collect data as needed andrequired, and provide aggregated report as required.
i. Strategize and implement methods that reduce mental health access barriers,disparities, and stigmatization that may impede patients' willingness to engage mentalhealth services.
j. Facilitate dialogue and listening skills in family and therapeutic environments for selectedcases, caregivers, and providers.
k. Coordinate events for family groups as needed to strengthen family self-confidence, self-esteem, resilience, and self empowerment.
l. Meet with the Mental Health Supervisor or Clinical Social Worker Consultant weekly toconsult and improve client engagement and services.
m. Provide training to ARP HVP NFP home visitors on mental health issues andintervention when requested
n. Attend mandatory training including but not limited to those required by Public Health,NFP national model, and the ARP funding. Training may include out of state training.
o. Attend needed training to provide mental health services to the ARP HVP NFP clientsand families. These training may include Mental Health First Aid (MHFA); CognitiveBehavioral Therapy (CBT); Trauma Informed Care; Postpartum Depression and/orMaternal Mental Health.
p. Collaborate with various stakeholders to promote and raise awareness of ARP HVPNFP.
q. Perform other duties as assigned.
