Queensbridge Health Advocacy Partners Program (HAPP): Full-Time Program Manager (temporary to permanent)

PROGRAM OVERVIEW: The NYC Public Health Corps (PHC) is a citywide investment in and commitment to the public health workforce and just recovery from COVID-19 with and for communities that have been disproportionately impacted. Following several years of COVID-19 specific work, the next phase of PHC aims to focus on chronic disease prevention and management through decreasing inequities in health and social outcomes for residents of public housing.
The Health Advocacy Partners Program, a component of PHC, will drive equitable pandemic recovery in priority public housing communities that have experienced longstanding health inequities. The program is funding community-based organizations to build up and utilize a workforce of community health workers (CHWs) to deeply engage with targeted NYCHA neighborhoods in preventing and managing chronic conditions like asthma, hypertension and diabetes. The program will work with NYCHA residents to help them better manage their chronic illnesses through:
· § One-on-one health coaching sessions
· § Individual and group-level educational workshops
· § Assistance in acquiring and navigating health insurance
· § Referrals to clinical and social support services in the community
· § Advocacy and community organizing
HAPP community partners will be working in the following neighborhoods and NYCHA developments:
Queens Long Island City Queensbridge (north and south)
Position Overview: Program Manager is responsible for leading, guiding, supporting, and monitoring delivery of health coaching services, social support referrals, engagement; participating in wellness activities geared towards achieving program goals; and meeting regularly with NYC Health Department to review data and program performance. The Health Program Manager will be expected to lead on multi-sectoral / health assessments, project planning, coordination, and support recruitment and procurement, at the field level. The program manager will be expected to mentor and/or capacity building with Community Health Workers and Supervisors. They will be responsible for the day-to-day operations of the health program, ensuing compliance with the award regulations and monitoring of health deliverables.
The major duties of the position include, but are not limited to:
· Manage the successful implementation of HAPP program model in Queensbridge houses.
· Supervise program supervisors and community health workers by tracking, collecting, and managing employment outcomes.
· With support from supervisors, drive performance and quality of programs against goals as they are implemented;
· Conduct quality assurance on all programs to ensure compliance with DOHMH and CMS requirements, fidelity to program models, and learning acquisition and satisfaction of trainees, as needed;
· Updating the organization's executives on project progress and estimated completion dates
· Create marketing content and materials (for general distribution and presentation on the CMS and NYCHA social media and in person)
· Creating a schedule and enforcing deadlines with CWH
· Overseeing the recruiting and hiring process
· Standardizing administrative procedures and emergency protocols
· With support from supervisors, identify learning and training opportunities for CHWs and work as a mentor and role model for less experienced staff.

- Proficiency in MS Word, Excel, PowerPoint, and Outlook
The recommended qualifications for this position are:
- Master’s degree in public health, public administration, social work, or a related field.
- Possess an understanding of community level work and the importance of collaborating and coordinating with other organizations.
- Effective communication and documentation skills.
- Cultural and linguistic competence for the target population; and
- Familiarity with motivational interviewing and trauma-informed care.
Salary rate of pay equivalent to $75,000 hourly (6 months)
