Creekside Post Acute is a small but extremely talented and tight-knit Skilled Nursing Facility in Yucaipa, CA. We are looking for a couple world class LVN’s to replace a few members of our LVN team who recently matriculated to nursing school. We are looking for LVN’s who are interested in Leadership, upward mobility and who desire a long & rewarding career in healthcare— including leadership opportunities or even pursuing an RN, RT, or even a PT/OT.

Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with procedures
- Collaborate with healthcare team members to develop and implement patient care plans
- Document patient information, including medical history, symptoms, and treatment progress
- Educate patients and their families on healthcare procedures and self-care techniques
- Ensure compliance with all relevant healthcare regulations and standards
- Maintain a clean and organized work environment
Qualifications:
- Valid Licensed Vocational Nurse (LVN) license
- Knowledge of medical terminology and procedures
- Experience in a medical office or healthcare setting preferred
- Familiarity with electronic health record systems (e.g., PointClickCare (PCC), Cerner)
- Ability to provide compassionate care to patients of all ages and backgrounds
- Strong communication and interpersonal skills
- Ability to work effectively as part of a multidisciplinary team

- Standards: Knowledge of healthcare regulations and standards to ensure compliance in patient care.
- Hospice Care: Experience or knowledge in providing end-of-life care for patients.

- Medical Office: Experience working in a medical office or clinic environment.
- Ventilator: Knowledge of ventilators and their operation.
- Lift: Ability to safely lift and move patients when necessary.
