The position serves as primary nursing staff for the ACT Team and may also support clients outside ACT team as well, depending on the need and capacity of the agency. The RN is responsible for medication monitoring and education; care coordination to include facilitation of linking with other health providers; administration of injections and working closely with MHMR prescriber to ensure adherence to treatment plan for each client. The position also helps clients navigate the medical system to access resources with the aim to improve patient outcomes, satisfaction, and continuity of care. Services are provided through a trauma-informed lens using a recovery-oriented model of care. Assertive Community Treatment is a team-based program that provides treatment, rehabilitation and support services to clients who have a history of multiple hospitalizations. ACT uses an integrated services approach merging clinical and rehabilitation staff expertise within one mobile service delivery system.
Experience: Should have experience in nursing related work. Prefer experience working with individuals with Mental Illness.
