TrueMed Home Health is seeking compassionate Registered Nurses (RN) for intermittent home health visits and case management throughout several counties. The successful RN will love working with home health patients in the actual home environment. The RN must be comfortable with documentation requirements, and have extensive experience in the health care industry. We are looking for self- directed RN's who are organized, responsible and comfortable with reassuring patients and their families regarding the disease process.
We have immediate Part-Time (per diem) opportunities with possibility to full time transition for Registered Nurses with recent Home Health experience.

We are currently hiring RNs in the Morgan Hill & Gilroy areas!!
Responsibilities and Duties
- Must be capable to schedule, open and handle a case promptly with submitting of all required paperwork within mandated 48 hours.
- Develop and/or follow an individualized Plan of Care that incorporates analysis of data and current scientific findings.
- Provide skilled interventions/treatments with a focus for achieving realistic patient outcomes within a specified time period.
- Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs.
- Facilitate and coordinate care among various disciplines including LVN's, PT/OT/ST/MSW and Home Health Aid's, as needed
- Administer medications and render treatment to home health patients under physician's orders, Plan of Care and Director of Nursing.
- Report changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).

- Full Time caseload is 5 to 6 visits per day when available
Qualifications and Skills
- 1-2 years experience as a Registered Nurse (RN) is preferred, but not required
- Current CPR and health clearance
- Current, active RN license in the State of California by the Board of Registered Nursing
- Excellent communication and interpersonal skills
- Knowledge of wound care/staging, IV infusions, blood draw, PICC & Peripheral line care is helpful
- Home health experience is a big plus, however, NEW GRADUATES are encouraged to apply (Some field training may be provided locally, some training may be held at the corporate office in South San Francisco)
- Must have reliable transportation and computer access
- Experience with Kinnser Software or other related EMR systems is a plus!
- Bilingual Preference (Russian, Mandarin, Cantonese, Tagalog)

Pay: $65.00 - $125.00 per hour
