About Impact
Impact Healthcare began in 2014 with a home health and hospice agency in San Francisco. We started with 32 team members, 89 patients, and a commitment to find, enjoy, and achieve a better way in healthcare—to have a positive impact on our team, patients, and community.
We now have locations across four states, providing home health, hospice, caregiving, remote patient monitoring, and pharmacy services to thousands of patients in their homes. We bring together services that allow patients to receive fantastic healthcare while staying home with their loved ones. And we are doing it by creating an environment that acts as a magnet for the most collaborative, unselfish and team-oriented people.
Although healthcare in the home has historically been thought of as “post-acute care,” we see it differently. Yes, our care helps people following an acute stay, but we also see it as “prevent-acute care.” Our services not only help people recover from emergent healthcare issues, but also prevent the recurring need for acute care in the first place. Our team adds quality and value to the patients we serve, their families, and the healthcare system by interrupting the cycle of going from one health crisis to the next and worrying about them in between. About the Role:
We are seeking a highly skilled RN Case Manager RNCM Hospice to join our team in the Health Care Services industry. As an RN Case Manager RNCM Hospice, you will be responsible for providing end-of-life care to patients in hospice care. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Your primary goal will be to manage patient care and ensure that patients receive the appropriate level of care based on their individual needs. You will also be responsible for coordinating care with other healthcare providers and ensuring that patients receive the necessary support and resources to manage their symptoms and improve their quality of life.
Minimum Qualifications:
- Active RN license in the state of California
- Minimum of 1 years of experience in hospice care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
Responsibilities:
- Provide end-of-life care to patients in hospice care
- Manage patient care and ensure that patients receive the appropriate level of care based on their individual needs
- Coordinate care with other healthcare providers
- Ensure that patients receive the necessary support and resources to manage their symptoms and improve their quality of life
- Maintain accurate and up-to-date patient records
Skills:
As an RN Case Manager RNCM Hospice, you will use your excellent communication and interpersonal skills to build relationships with patients and their families. You will also use your strong organizational and time management skills to manage patient care and ensure that patients receive the appropriate level of care based on their individual needs. Your ability to work independently and as part of a team will be essential in coordinating care with other healthcare providers. Additionally, your experience with electronic medical records and working with diverse patient populations will be beneficial in providing the highest quality of care to our patients.
Job Types: Full-time, Part-time, Per diem
Pay: $45.00 - $60.00 per hour
Expected hours: 9 – 40 per week
