RN-Nurse Manager

FLEXIBLE SCHEDULE; Four 10's
UBER provided at company expense within Queens
NO NIGHTS, WEEKENDS or HOLIDAYS
- Uber access to patients once in borough (if you live in Queens, you can take uber from home to first visit, last visit back to home)
- 0.25 management, 0.75 clinical/patient facing (home visits)
-Patients: combination of high acuity patients that need to be seen once/month, sick/urgent visits, post- hospitalization visits
- Not a traditional RN Case Manager Role (no routine skilled nursing tasks expected apart from as needed phlebotomy, wound care for educational purposes, potential as needed catheterization; we routinely refer to VNS for ongoing/long-term skilled nursing)
Typical Visit: Often shared with our Palliative Care Nurse Practitioner, routine nursing assessment (vital signs, focused palliative review of systems, physical exam, goals of care conversations)

Responsibilities
- Works independently and in collaboration with the Director of Clinical Operations in onboarding, training and managing the CCM and Palliative Care program including the field Nurses, LSW's, and CHW's.
- Provides leadership expertise in problem solving, mission/practice standards, job performance skills, and communication.
- Delivers ongoing coaching, audits, and performance reviews of the Registered Nurse, CHW's, and LSW's.
- Works with leadership and the recruiter to recruit and hire Palliative Care providers.
- Communicates organizational policies and procedures, evidence-based best practices, and regulatory/industry changes to direct reports.
- Supports direct reports in their achievement of individual professional growth goals through coaching and mentoring, evaluation, and collaborative performance improvement expectations.
- Collaborates with other leaders and teams to foster clinical excellence, fiscal stewardship, employee development and retention, and service growth
- Ensures compliance with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Monitors and evaluates employee performance. Holds employees accountable for the attainment of defined goals and objectives. Executes effective performance redirection when needed.
- Performs other duties as assigned including patient visits
Qualifications
Required:
- Bachelor of Science in Nursing (BSN) degree.
- Licensed as a Registered Nurse in the state of practice.
- Registered Nurse located in New York City.
- Three (3+) years of palliative care/geriatrics experience as a Registered Nurse.
- Two (2+) years of leadership/clinical management experience.
- Experience working in an interdisciplinary team.
Preferred:
- Home care experience.
- CPR certiication.
- ACLS certification.
