SUMMARY: Provide and manage health education programs that help individuals, families, and their communities maximize and maintain healthy lifestyles. Use data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments.
POSITION QUALIFICATIONS:
× High School Diploma or High School Equivalency (GED)
× Enrollment at Itawamba Community College
× Excellent oral, written and interpersonal skills as well as good customer service skills.
× Valid MS Driver’s License and continuous automobile insurance
ESSENTIAL DUTIES:
× Enroll in Community Health Worker Certification program at ICC and maintain a C average in all classes
× Answer telephone in a police and courteous manner.
× Greet patients as they enter clinic and assist patient in signing in.
× Obtain patient information at time of visit.
× Make appointments and referral appointments as requested.
× Review and validate patient insurance and obtain any authorizations from insurance as needed.
× Collect patient balances for visit and account
× Serve with other members of the medical home team in providing primary health care that is relationship-based with an orientation toward the whole person.
× Prepare and distribute health education materials, such as reports, bulletins, and visual aids, to address smoking, vaccines, and other public health concerns.
× Develop and maintain health education libraries to provide resources for staff and community agencies.
× Document activities and record information, such as the numbers of applications completed, presentations conducted, and persons assisted.
× Maintain databases, mailing lists, telephone networks, and other information to facilitate the functioning of health education programs.
× Develop educational materials and programs for community agencies, local government, and state government.
× Develop and maintain cooperative working relationships with agencies and organizations interested in public health care.
× Collaborate with health specialists and civic groups to determine community health needs and the availability of services and to develop goals for meeting needs.
× Develop and present health education and promotion programs, such as training workshops, conferences, and school or community presentations.
× Develop operational plans and policies necessary to achieve health education objectives and services.
× Design and conduct evaluations and diagnostic studies to assess the quality and performance of health education programs.
× Provide guidance to agencies and organizations on assessment of health education needs and on development and delivery of health education programs.
× Perform other duties as assigned
