Pathways to Wellness Medication Clinic is an outpatient mental health treatment program founded in 2000 to meet a significant need within our mental health community. Pathways to Wellness is designed to provide a vital link in the spectrum of services available to adults and children requiring mental health management. Staffed by Board Certified Psychiatrists, Nurse Practitioners, Pharmacists, Physician Assistants, Psychiatric Nurses and Licensed Clinicians, our clinics provide services that are intended to afford significant relief from the symptoms of mental illness, to facilitate our patient’s ability to return or remain in the community, and to avoid treatment at higher levels of care. Pathways to Wellness has provided over 25 years of specialty mental health services to individuals in Alameda County community.
Case Manager position is based out of our downtown Oakland Clinic (right in front of the Bart station and bus stop easy for commuting; no car needed). We are looking for Case Managers who are compassionate and culturally competent and are interested in providing support and assistance to individuals facing mental health challenges. The ideal candidate will possess a minimum of a Bachelors degree in Social Work or Psychology. OR a minimum of 3 solid years of experience in social work/case management. Masters degree in Social Work preferred. Licensed or Registered Intern (ASW) a plus!
Also looking for individuals with documentation skills ability to write daily progress notes and comprehensive treatment plans. And must have experience working with adults!
*This is an "in office" case management position. Lots of phone coordination, brokerage, linkage, connecting clients with community resources and follow-up with outside agencies.
Clinic hours are M - F 8:30am-5:00pm; some evenings shifts available, based upon client needs.
General Responsibilities:
Our client population is persistently mentally ill adults. We also have children and adolescents. Case Managers will be expected to help clients apply for benefits as well as coordinate care and transition with outside providers. [i.e. secure housing, AOD services, PCPs, therapists, employment agencies, etc.] This position requires daily outreach via phone to clients who did not show up for scheduled appointment.
There is a daily productivity requirement for the Case Manager position. Under direction performs professional social work to approximately 10-20 clients and/or families daily to obtain housing, health, financial and social services; assist in finding Primary Care providers; provides assessments and develops and implements case management plans; performs related duties as required. Daily documentation required.
- Experience working with adult clients who have a serious and persistent mental illness in mental health care setting.
- Experience in case management and in coordinating services for clients.
- Excellent interpersonal and client engagement skills and experience.
- Knowledge and understanding of records charting for Medi-Cal reimbursement.
- Excellent communication skills, verbal and written.
- Ability to work with minimal supervision.
- Ability to work well with physicians, nurses and other clinicians. Must work as a team player.
- Ability to handle multiple demands and /or crisis situations in a professional manner.

- Completely comfortable, competent in using Electronic Health Record software.
