Overview:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will provide comprehensive patient care and support while coordinating with healthcare professionals to deliver high-quality services.
Duties:
- Develop and implement individualized care plans for patients
- Coordinate patient care activities with interdisciplinary team members
- Conduct assessments and evaluations to determine patient needs
- Monitor patient progress and update treatment plans as necessary
- Educate patients and families on healthcare management strategies
- Collaborate with physicians and specialists to optimize patient outcomes
- Ensure compliance with regulatory standards and ethical practices
Qualifications:
- Valid RN license and BSN degree required
- Proven experience in healthcare settings such as acute care, primary care, or emergency medicine
- Strong knowledge of medical-surgical procedures, physiology, and patient care techniques
- Experience in behavioral health, geriatrics, or working in a Level I Trauma Center is a plus
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment while maintaining attention to detail

Pay: $50.23 - $53.52 per hour
Expected hours: No less than 40 per week
