Regional Hospice is looking for licensed Registered Nurse to work Per Diem with patients in the Community.
We offer a generous 401K Plan with a 4% Match for every 5% contributed with immediate vesting.
This position needs a candidate who is skilled in pain and symptom management, is able to confidently make assessments over the phone, and is able to triage patient needs and assign other nurses (or self) to make patient visits as appropriate.
Tasks:

Provides skilled and supportive nursing care to patients and families

Performs assessment of patient and family, through observation, interviews, and analysis of records

Responds to physical, emotional psychosocial, environmental, needs as warranted.
- Completes documentation required by agency, government, and third-party systems

Identifies patient and family needs for other home health care services and makes referrals through agency-designated channels

Identifies signs and symptoms of the disease and explains diagnosis and the nature of palliative care to the patient and family as appropriate

Utilizes appropriate nursing interventions to alleviate pain and discomfort in accordance with the plan of care.

Facilitates and supports decision making by the patient and primary caregivers

Graduation from an accredited school of nursing; BSN preferred

Currently licensed or the ability to obtain a license to practice as a Registered Nurse in the State of Connecticut.

A minimum of two years of nursing experience, preferably in medical/surgical nursing required

Valid Clean Driver’s License Required

Home Health Care and/or Hospice / End of life care experience preferred

Active client contact within past three years, preferred.
- Certified Hospice and Palliative Care Nurse (CHPN) preferred
