Responsibilities:
- Provide compassionate and comprehensive care to patients in hospice settings
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Conduct thorough assessments of patients' physical, emotional, and psychosocial needs
- Administer medications, including injections, as prescribed by the physician
- Monitor and evaluate patients' symptoms and response to treatment
- Educate patients and their families on disease management, symptom control, and end-of-life care options
- Coordinate with other healthcare providers to ensure continuity of care
- Assist with discharge planning and coordination of services for patients transitioning from hospital to home or other care settings
Skills:
- Knowledge of discharge planning processes and resources
- Ability to triage patient needs and prioritize care accordingly
- Familiarity with administering injections and medication management
- Strong communication skills to effectively interact with patients, families, and interdisciplinary team members
- Compassionate and empathetic approach to end-of-life care

Pay: $45.00 - $60.00 per hour
