Our client in Santa Clara is looking for multiple Physical Therapists in Santa Clara.
Position Summary:
The Physical Therapist's role is pivotal in restoring function and minimizing disability following disease or injury, ensuring clients reach their maximum performance within their capabilities. This role involves evaluating and treating patients using medically prescribed physical therapy programs and may include supervising Physical Therapy Assistants.
Responsibilities:
- Conduct initial admission/evaluation visits and notify the clinical team manager of assessment findings promptly.
- Develop and revise comprehensive care plans in consultation with the clinical team manager.
- Document observations, interventions, goals, and discharge plans consistently.
- Assess and coordinate the need for additional services from the interdisciplinary team.
- Provide skilled care, interventions, and patient education during visits.
- Obtain and document physician orders and supervise Home Health Aides as per regulations.
- Communicate effectively with the clinical team, physicians, and other stakeholders regarding patient status and needs.
- Adhere to documentation and technology standards and meet productivity goals.
- Actively participate in team conferences, in-service programs, and quality improvement initiatives.
- Assist in the development of agency protocols, procedures, and policies as required.
- Conduct patient education and supervise other agency personnel as needed.
Qualifications:
- Graduate from an accredited physical therapy program approved by the American Medical Association and the American Physical Therapy Association.
- Possess a valid California license as a Physical Therapist.
- Minimum of 2 years of recent experience as a Physical Therapist, preferably with a focus on gerontology and home care.
- Current CPR Certificate.
- Valid California driver’s license and automobile insurance.
- Proficiency in electronic input and telecommunication devices.

Essential Physical and Mental Requirements:
- Physical: Ability to drive to patients’ homes, maneuver around objects, lift patients of varying weights, and perform necessary tasks during home visits.
- Sensory: Ability to effectively communicate verbally and in writing.
- Mental: Ability to understand and implement clinical directions, make decisions related to patient care, and demonstrate critical thinking skills consistent with standards and regulations.
