JOB DESCRIPTION:
The individual in this position assumes day-to-day responsibility for receiving intake information and assists with coordinating the paraprofessionals’ schedule
JOB RESPONSIBILITIES:
- · Answers phones, receives all intake information for paraprofessional cases and enters the appropriate information in the computer.
- - Completes intake form, routes it appropriately for admission approval.
- - Assists with scheduling appropriately skilled person to fill the case; inputs scheduling information on the computer.
- -Assists with schedule any replacement workers needed; inputs scheduling information on the computer.
- - Communicates with clients/families on Home Health Aide-related issues
- - Takes telephone referral information
- - Handles all matters related to these personnel and the clients they serve, with the exception of issues that require professional/clinical judgment
- - Assists in maintenance of adequate staffing patterns and time-off records for staff
- - Helps Coordinates the scheduling of all field staff employees
- - Maintains case management notes that document communication that relates to client care and how each situation was handle
- - Checks cases after filling and weekly thereafter
- - Keeps careful records of all cases and aides
- - Answering questions from patients and providers, responding to phone and email inquiries
- -Verifying health insurance coverage and managing health insurance issues
- - Assists with transferring field staff from one case to another, where appropriate
- - Performs other duties as required
POSITION QUALIFICATIONS:
- · Must be a high school graduate or have a GED
- · Prior experience in a related field (home health) with Intake experience in home health preferred
- Must have strong attention to detail
- Must have ability to type 50 WPM with knowledge of English rules of spelling grammar, and punctuation
- Read, write, process information, communicate accurately to others and establish priorities
- Must have reliable Transportation
- Must have Social Security Card or Birth Certificate
- Must be authorized to work in the USA
- Bilingual in Spanish required
- Knowledge of Word, Windows and Kinnser/WellSky software
- Requires ability to read, write, process information, communicate accurately to others and establish priorities
- Requires knowledge of home care law and regulation
- Responsible for receiving and processing new patients' information and setting up admissions
- Insurance verification
*We are required by management to report the outcome of all interviews to the Texas Workforce Commission*
Pay: $14.00-16.00 per hour (depending on experience)
Schedule: Monday- Friday 8:30am -5pm
