GENERAL DESCRIPTION
The position is responsible for the delivery of professional occupational therapy services in the patient’s home, which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each patient and provide home modifications and inspections for ADA compliance. The position is also responsible for providing implementation of Independent Living and other skills training to ensure that individuals with disabilities maximize their opportunities for independent living.
ESSENTIAL JOB FUNCTIONS
1. Complete home assessments with clients and/or caregivers to determine ways to improve safety and independence in the home
2. Generate personalized reports for clients and/or caregivers to provide them with basic home modifications and activity modifications
3. Educate clients/caregivers on basic fall prevention strategies and techniques
4. Establish rapport and engage in a collaborative discussion with clients and/or caregivers to meet their individual needs
5. Evaluates home environment hazards or barriers to more independent living. Identifies equipment needs.
6. Write an assessment report after the home visit, documenting findings and recommendations.
7. Assists patients in achieving optimum levels of functioning in self-care, work, school, homemaking and/or leisure activities.
8. Under the direction of the physician, plans a therapeutic program to develop the patient’s motor, cognitive, sensory-integrative, psychological and social function needs.
9. Perform an accurate OASIS-C timepoint(s) according to CMS guidance
10. Participates in the development of the total plan of care.
11. Will utilize physical means of sound, light, electricity, water, heat, cold, exercise, splinting, and manual interventions.
12. Develops a treatment program and establish goals for improved function. Communicates plan of care to referring physicians and other team members.
13. May teach new skills or retrain patients in once familiar daily activities that have been lost due to illness or injury.
14. Responsible for documenting care plan, rehabilitative potential, and discharge planning needs. 15. Regular and predictable attendance
REQUIRED SKILLS
- Graduate from accredited school of occupational therapy
- Current Missouri Occupational Therapist (OTR) license
- Licensed as OTR for a minimum of two years with two years’ experience.
- Possesses and maintains current CPR Certification
- Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
- Knowledge of ADA requirements and Home Modification for Accessibility

Job Type: Full-time
Pay: $38.00 - $48.00 per hour
