Influences outcomes by providing expert consultation to all care providers and implementing improvements in health care delivery systems.
This Advanced Nurse Specialist (ANS) will be an integral member of the Adult Complex Care team. This team provides complex care management for defined populations of adult patients within the UCSF health system in a primarily remote model. These populations include the most medically and psycho-socially complex patients, high risk or high utilizing patients, those with poorly controlled chronic conditions, those who belong within a specific ACO contract, or other types of groups that might benefit from more intensive support as an extension of UCSF Primary Care or Specialty Care. The Adult Complex Care clinical team is comprised of ANSs and Licensed Clinical Social Workers (LCSW). As high-level professionals, the complex clinical team is expected to play a role in program development, refinement, and evaluation. Clinically, this ANS will be responsible for collaboration with the LCSWs in completing a comprehensive intake assessment on each patient followed by development of a complex care management plan. A team of consultants including a physician, pharmacist, and a behavioral health professional supports the complex clinical team. During interdisciplinary team case conference, cases and evidence-based care plans are presented and refined based on recommendations made by the team. The ANSs and LCSWs will implement the care plans with ongoing care management for their panel of high risk patients including high level care coordination between primary care providers and specialists, medication reconciliation, patient education, patient and caregiver support, and smoothing transitions between sites of care. Motivational Interviewing techniques are employed in supporting patients’ efforts to identify and work toward their health care goals with improved self-management of their chronic conditions.
The ANS provides health assessment, medication reconciliation, patient education, and treatment recommendations, serving in a consultative role to primary care or specialty care providers. In addition, this ANS will serve as a clinical resource to other members of the Clinical Programs in the Office of Population Health, who may be providing interventions such as screening, health coaching and care coordination to identified high-risk cohorts of patients with chronic illnesses. This position within the Adult Complex Care team is critical in efforts to expand access for patients requiring tertiary and quaternary levels of care, by improving outcomes for patients served by UCSF Health, while striving toward the “quadruple aim” of healthcare delivery reform: improved clinical outcomes, better patient experiences, lower costs and improved provider satisfaction.
This ANS role is part of a dynamic Care team within the Office of Population Health (OPH) Clinical Programs Care. This position will report to the Manager, Complex Care. The ANS is a fully remote position with some requirement for travel to San Francisco or the Brisbane Office to meet Divisional priorities.
The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $209,447 - $235,840 (Annual Rate).
