*JOB DESCRIPTION:*
The Case Manager II under the direction of the Assistant Director of Supportive Housing is responsible for ongoing needs assessment, individual treatment plans, implementation, support, coordination of care, information, referral and advocacy for individuals who are in the Providence House Supportive Housing facility. Residents in our supportive housing program are individuals living with serious mental illness, and/or have a history of substance abuse as well homelessness. The position requires successful working relationships with residents that support their ability to increase their self-reliance and independent living skills. The Case Manager II will be responsible for influencing and recognizing positive behavior changes and identifying problems and obstacles that threaten to undermine the resident’s goals. Keep clear and accurate files and records are essential to this position. This position will require occasional evening and weekend hours.
*REPORTS TO:* Assistant Director of Supportive Housing
*RESPONSIBILIITES:*
*Client/Service Delivery*
* Maintain a caseload of 17 to 21 individuals. Meet biweekly with residents to develop and monitor each individual’s service plans.
* Facilitate group and individual services that are client centered, strength-based and trauma-informed to clients who are struggling with SMI (Serious Mental Illness) and/or SUD (Substance Use Disorder) and promote recovery.
* Complete strength-based comprehensive needs assessment that will inform individualized service plan.
* Provide client-centered services that support independent living skills.
* Provide crisis management and intervention.
* Conduct resident apartments to ensure that they are properly maintained.
* Have working knowledge of community resources and refer clients for needed services.
* Work with Clinical Social Worker and Assistant Director to ensure that residents are linked to services such as mental health, substance use, education/vocation and employment.
* Attend and utilize trainings to increase knowledge of effectively meeting client’s needs through evidence-based practices.
* Asses client mental health and recovery needs.
* Monitor client access to and involvement with treatment and collaborate with providers to help address barriers to treatment and the recovery process.
* Perform other duties as assigned by the supervisor.

* Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violence and trauma sensitive culture.
* Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community).
* ntegrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.
*Team/Staff Relations*
* Work as a Providence House team member with all staff and volunteers in order to implement the mission and core values of the agency.
* Attend regular internal and external agency meetings and trainings.
* Work as part of team to plan and develop resident programs.
* Administrative
* Keep accurate, up-to-date documentation for all clients. Including but not limited to, service plans, case notes, assessments, etc.
* Complete all paperwork in a timely and accurate manner that ensures compliance with stakeholders.
* Assist with any and all data collection systems.
*REQUIREMENTS*
* Candidate must have a Bachelor’s degree in Social Work or related field (Master’s Degree preferred).
* Three to five years of experience working with the formerly homeless, mentally ill population required.
* Experience with long-term homeless, low income and diverse populations.
* Have knowledge and experience of major psychiatric disorders and substance use disorder.
* Excellent verbal and written communication skills and the ability to work independently.
* Strong interpersonal and engagement skills.
* Strong organizational and time-management skills.
* Excellent written and verbal communication skills.
* Competent technology management skills, including proficiency with using Microsoft Office; client database management.
*WORK SCHEDULE*
* The Case Manager II position is a full time, salaried position. Hours worked will include one late evening weekly and occasional weekends.
* The location for this position is in Brooklyn, New York.
*SALARY AND BENEFITS*
Salary range $60K - $70K Annually. Paid time off benefits include: 14 paid holidays, 3 weeks of vacation, 3 personal days, 12 sick days, early closure proceeding holidays, and a day off annually in recognition of your Birthday.
