The State of New Hampshire, Department of Health and Human Services, Bureau for Family Centered Services has a full time vacancy for Nurse Consultant.
SUMMARY:
To plan, coordinate and evaluate program activities in providing consultation services to community-based and health agencies in the development and organization of care coordination services for children with special health care needs and their families for the Bureau for Family Centered Services.
RESPONSIBILITIES:
Works in collaboration with program staff, care coordinators, local providers, educators, health providers, and service agencies on specific health concerns or disease processes for children and youth with special health care needs.
Reviews medical reports and writes health and social history summaries/evaluations which inform the development of individualized care plans and communicates these recommendations to local care providers, families and service agencies.
Develops and conducts educational opportunities and programs related to children with special health care needs, working in cooperation with federal, state and local agencies.
Establishes, analyzes, evaluates, and revises goals and objectives for programs and services provided to children with special health care needs and their families.
Analyzes, develops and maintains protocols, policies and procedures for best practice related to care coordination for children with special health care needs and their families.
Participates in the training of nursing staff and provides training, guidance and instruction to staff, care coordinators, local providers, educators, health providers, and service agencies engaged in the care and treatment of children with special health care needs.
Works with Medicaid enrolled children with severe disabilities and their parents/caregivers to make the best use of available health care coverage from third party carriers and the Medicaid Program. Analyzes statistical, programmatic and financial data and develops fiscal impact information as they pertain to the coordination of benefits and the operation of Health Insurance Premium Payment program.
MINIMUM QUALIFICATIONS:
Education: Master's degree from an accredited college or university program with a major study in public health nursing, nursing education, or public health or other nursing specialty. Each additional year of approved formal education may be substituted for one year of required work experience.
Experience: Four years of nursing experience, two years of which included practice, under supervision in the specialized field in which to be employed, such as Public Health Nursing, Maternal and Child Health, Mental Health or Nursing Education, and two years of which included supervisory nursing or teaching experience. Each additional year of approved work experience may be substituted for one year of required formal education at the graduate level only.
License/Certification:
1. Current license as a Registered Nurse in New Hampshire.
2. Current certification in CPR.
3. Valid New Hampshire driver’s license and/or access to transportation for use in statewide travel.
SPECIAL REQUIREMENTS: Direct experience/familiarity with children with special health care needs and the health care delivery system for this population of children. Broad knowledge of software applications and specific knowledge of Excel and other Microsoft Office systems for data collection and analysis.
PREFERRED QUALIFICATIONS: Experience in Pediatrics or School Nursing preferred.
