The Case Manager in the Mental Health Division fulfil a vital function for clients and families by working with them under the guidance of the therapist, and in collaboration with other members of the treatment team. Case managers work to build a trusting and productive relationship with the client/family, and to provide the support and resources needed to achieve identified treatment goals and improve his/her quality of life.
Position Qualifications/Requirements
Case Manager: Specifically for DTI - Must work onsite Monday - Friday. Schedule 11:30am-8:00PM.
1. Two years of college in a related mental health field.
2. Three years’ experience providing direct services to seriously emotionally disturbed children in a school, residential program, day treatment, mentoring program, shadowing or similar setting.
3. Possession of valid Class C or higher driver’s license and insurability by the agency insurance carrier(s). An acceptable alternate means of transportation necessary to the completion of one’s duties may be considered.
4. Must possess basic computer skills.
Rehabilitation Specialist:
1.bachelor’s degree in social work, psychology or a related field, from an accredited College or University, and
2.One year in providing direct services to seriously emotionally disturbed children in a school, residential, day treatment, mentoring, shadowing or similar setting, or
3. Associate’s level degree and six years’ experience in a mental health setting of physical restoration, social adjustment, or vocational adjustment.
- Possession of a valid Class C or higher driver’s license and insurability by the agency insurance carrier(s). An acceptable alternate means of transportation necessary to the completion of one’s duties may be considered.
Physical Demands:
· While performing the duties of this job, the employee regularly operates a computer
and other office machinery. Communicates with others in the office environment and outside the office environment.
· The employee frequently works in outdoor setting and is exposed to outdoor weather conditions.
Work Environment:
· Outdoor and outdoor in field.
Duties and Responsibilities
Key Tasks:
- Trains family/caregivers in specific interventions which includes behavior modification, parenting skills, and other skills and strategies that assist in achieving identified goals.
- Trains the child in strategies and techniques to deal with specific problems of daily living, i.e., anger, social isolation, aggression, etc.
- Provides services as contained in the Service Plan in the location specific to the needs of the client/family (Home, Community, Other etc.).
- Reinforces safety guideline and protocols related to clients, families and community in accordance with agency policies and procedures, and governing regulations.
- Accompanying child and family as needed to necessary appointments (Court, DCFS, Probation, Medication etc.) and providing transportation when needed.
- Provides services in the home and community, whenever and wherever convenient to client/family.
- Demonstrate skills used in acting as a liaison between the client and their families, teachers, etc. in an effort to enlist their interest and cooperation to provide quality of care.
- Providing referral and linkages to local agencies and community resources based on the identified needs of the client.
- Facilitates the navigation of systems related to housing, social welfare, educational to address the individual needs of clients, and assist in improving access.
- Provides advocacy for clients and families related to rights and responsibilities with local, state, federal systems including, DMH, DCFS, Probation, Education systems, and other institutions such as courts, and/or medical appointments.
- Participate in intra/inter agency conferences and/or consultations with other treatment team members regarding overall service delivery and other concerns/issues.
- Complete documentation in the medical record for all direct service activities claimed.
- Participates in quality assurance and continuous quality improvement (Peer Review, ATS etc.)
- Other duties as designated by supervisor, manager and/or program head.
