Job Title: Intake Registered Nurse (RN) - PACE Organization
Reports To: RN Supervisor
FLSA Status: Non-Exempt
Job Summary: The Intake Registered Nurse (RN) for a PACE (Program of All-Inclusive Care for the Elderly) Organization is responsible for managing the intake process for new participants. This role includes assessing potential participants’ eligibility, conducting initial health evaluations, obtaining past medical records, medication reconciliation, coordinating with interdisciplinary team members for their input, completing the Enrollment Agreement and being a community ambassador in the community for the PACE program. This role also works with the DHCS to upload material to the state site, works closely with Quality and Compliance and IDT.
Key Responsibilities:
- Initial Assessment:
- Conduct comprehensive initial health assessments for potential participants, including medical history, physical examinations, and collecting information on functional status.
- Evaluate participants’ eligibility for the PACE program based on established criteria.
- Coordination and Communication:
- Partner with additional intake team members to follow up and communicate with potential participants.
- Answer informational calls and explain program to interested people.
- Prepares report for interdisciplinary team members, including dietician, transportation, physicians, social workers, OT, PT, day center staff and others for input on the potential participant.
- Act as a liaison between participants, families, and the PACE organization to facilitate clear and effective communication until the Enrollment Agreement is assigned, and the IDT assumes this responsibility
- Documentation and Compliance:
- Complete and maintain accurate documentation of all assessments, medical records, other forms of documentation, and communications in compliance with regulatory standards and organizational policies.
- Ensure all required documentation for new enrollments is completed and filed appropriately and timely and follows HIPPA guidelines.
- Education and Support:
- Provide education and support to potential participants and their families about the PACE program, including benefits, services, and processes.
- Address questions and concerns from participants and their families regarding the intake process and ongoing care.
- Process Improvement:
- Participate in quality improvement initiatives to enhance the intake process and overall participant satisfaction.
- Identify areas for improvement and collaborate with the admissions team to implement changes.
- Compliance and Regulations:
- Stay current with federal, state, and local regulations related to PACE programs and ensure all intake processes are compliant.
- Participate in training and professional development to maintain and enhance nursing skills and knowledge.
Qualifications:
- Education:
- Associate’s or bachelor’s degree in nursing (RN) from an accredited nursing program.
- Current, valid RN license in the state of California.
- Experience:
- Minimum of 2 years of nursing experience, preferably in geriatrics, community health, or a related field.
- Experience with intake, admissions, or case management in a healthcare setting is highly desirable.
- Skills and Abilities:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within an interdisciplinary team.
- Proficiency in electronic health records (EHR) and other healthcare-related software.
- Strong organizational and time management skills.
- Other Requirements:
- Valid driver’s license and reliable transportation for home visits and assessments.
- Ability to work flexible hours, including occasional evenings or weekends, as needed.

§ Physical demands: Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
§ Work Environment: Employee must be able to work in a busy and demanding clinic environment with moderate to loud noise levels.
