Position Overview:
As a Medical Office Assistant, you will be assisting in care coordination for patients who are
returning home from nursing homes or rehabilitation facilities. This job entails extensive telehealth communication with patients, as well as colleagues and management, therefore, being personable, compassionate, and understanding are important traits. This position reports to the Director of Care Management, unless otherwise assigned.
General Responsibilities:
● Researching & coordinating various services such as at home therapies, and specific to each
patients’ needs. Some examples include:
● Coordinating with PCP or specialist providers’ offices
● Gathering, uploading, and sending patient correspondence to facilitate and/or
follow up on care.
● Complete insurance/benefit applications (e.g., Medicaid; SNAP)
● Follow up on applications via phone/email
● Complete Meals on Wheels applications
● Assisting with ordering medical supplies
● Securing medical transportation to/from appointments
● Extensively communicate with health providers, patients, and key contacts, to ensure
continuity of services.
● Document patient notes & care planning activities in the electronic health record in an
accurate, timely, and detailed manner for services researched/provided.
● Follow-up on phone calls, emails and correspondence promptly to ensure quality standards
of service delivery are met.
● Uphold the agency’s protocols, policies & procedures to ensure compliance with treatment
planning.
● Perform other related duties as assigned.
Requirements and Qualifications:
● Active, unrestricted certification as a Medical Office Assistant.
● Strong communication skills, team oriented approach.
● Well-versed in computer skills. Ability to multitask.
● Superb customer service.
● Reliable & dependable.

Pay: $20.00 - $22.00 per hour
