SUMMARY OF POSITION RESPONSIBILITIES:
The Case Manager uses a comprehensive strategic model of strength-based case management focused on court accountability, holistic treatment planning, SMART goals which lead to successful client outcomes and community awareness. Case managers/counselors provide each student with intensive supervision and a supportive network, which enables the student to meet the expectations of the court, decrease the likelihood for re-offending and making them self - conscious agents of their own liberation.
ESSENTIAL DUTIES:
1. Provides rehabilitation services aimed at decreasing drug use, violent and criminogenic behaviors.
2. Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring staff performance.
3. Admits new clients by reviewing records and applications; conducting orientations.
4. Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
5. Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
6. Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
7. Maintains clients’ records by reviewing case notes; logging events and progress.
8. Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
9. Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
10. Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.
11. Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
12. Meets budget by monitoring expenses; implementing cost-saving actions.
13. Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics; completing grant and subsidy applications.
14. Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
15. Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
QUALIFICATION REQUIREMENTS:
1. Bachelor’s degree in Criminal Justice or related field or minimum five years paid work experience in Substance Abuse/Criminal Justice or Certified in CCAAP, CAADE, etc. Minimum of three years of therapeutic group work.
2. Technical Skills: Strong computer skills required.
3. Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, report or schedule form. Use tact, discretion, initiative and independent judgment within established guidelines.

Pay: $21.00 - $31.00 per hour
