ThedaCare & First 5 Fox Valley are seeking a part-time Help Me Grow Early Childhood Family Resource Navigator to join a team of people dedicated to improving the lives of children living in our community. Job Summary: The Help Me Grow Early Childhood (EC) Family Resource Navigator will support ThedaCare healthcare providers working with families that have children ages 0-5. The Help Me Grow EC Navigator will support families by connecting them with community resources and referral activities specific to early childhood developmental or behavioral concerns that might be identified through developmental screening or within the social determinants of health risk factors. The EC Navigator will work with partner Help Me Grow (HMG) outreach navigators to support a broad range of family needs such as food insecurity, housing, transportation, parent education, and child care, among others. The EC Navigator will follow up with patients using the HMG system and provide follow up to assure families are connected with the resources needed. The EC Navigator will be a member of the Help Me Grow (HMG) navigator community team and will document case information in EPIC and the HMG database, and will build relationships with community-based providers. Oversight will be provided daily by the SDOH Navigator Supervisor; additional support provided by the HMG Rural Health Lead Navigator, Community Health Specialist and First 5 Executive Director.
Early Childhood Family Resource Navigator Functions & Responsibilities:
§ Manage incoming calls, emails, and other messages and accurately record family intake information in EPIC and Help Me Grow (HMG) data systems.
§ Throughout the referral process, maintain confidentiality and build trusting relationships with families to identify and support concerns and/or needs while documenting referral and follow up resources provided in the data systems.
§ Utilize ASQ (Ages and Stages Questionnaires) screening tools, EPIC and the Help Me Grow Databases, and coordinates follow-up conversations with families, data entry, screen reminder system management, and reporting.
§ Provides system navigation for families, researches most appropriate resources and education and information on development, behavior management and services, and provides advocacy and follow-up as needed.
§ Coordinates patient information and communication between and among the patient/family, the referring/accepting facilities, community navigator and communicate to caregivers as applicable to ensure smooth transition of care.
§ Conduct follow-up with families via phone, mail and/or email to ensure effectiveness of referral and provide additional support.
§ Participate in initial and ongoing training.
§ Assists with program outreach activities, including training and educating rural providers and clinicians on what HMG and SDOH are, how the team works, and how they can refer rural patients for care.
§ Attends staff meetings, conferences, and workshops regularly to upgrade professional skills and understand systems and current best practices for working with children and families.
§ Other duties as assigned including, but not limited to, supporting other F5FV projects, initiatives, and promote Help Me Grow within targeted communities.
Education & Experience Required:
§ Bachelor’s Degree in special education, early childhood development, social work, nursing or related health or human services field, plus one-year experience working with children and families; OR Associates Degree in early child development, or related health or human services field, plus three years’ experience working with children and families.
§ Knowledge of typical and atypical child development and special needs conditions highly desirable
§ Bilingual, English/Spanish highly desirable
§ Ability to use computer technically well to complete job functions, including search features of a database, Excel, Word, PowerPoint, attach a PDF to an email, use template letters, manage outreach schedule, and input and analyze client data proficiently
§ Well-developed communications skills, both written and verbal, along with outstanding listening skills
§ Demonstrated capacity for both independent work and collaboration within a team

§ Compassionate personality with the ability to interview individuals to obtain information, assess client needs and provide appropriate information and referrals, and maintain confidentiality of all information.

§ Good time management and organizational skills, with ability to work with limited supervision.
§ This position requires access to a car and a valid driver’s license and will require daytime travel throughout Wisconsin.
§ Must pass a criminal background check prior to employment offer and periodically thereafter.
