Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a passion for patient care and possess strong skills in mental health counseling, patient assessment, and medical records management.
Duties:
- Conduct patient assessments to determine needs and develop individualized care plans
- Coordinate with healthcare providers to ensure comprehensive care delivery
- Maintain accurate and up-to-date medical records for each patient
- Collaborate with interdisciplinary teams to provide optimal care for patients, including pediatrics and trauma cases
- Conduct diagnostic evaluations and implement appropriate interventions
- Manage inpatient and outpatient cases with a focus on behavioral health support
Requirements:
- Bachelor's degree in Nursing, Social Work, Psychology, or related field; Master's degree preferred
- Experience working in a Level II Trauma Center or Emergency Medicine setting is advantageous
- Strong background in patient care and mental health counseling
- Proficiency in conducting patient assessments and developing care plans
- Excellent organizational skills with the ability to manage medical records effectively
