Duties:
- Provide skilled nursing care to patients in their homes or other healthcare settings
- Assess patient needs and develop individualized care plans
- Monitor and evaluate patient progress and adjust care plans as needed
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date medical records
- Adhere to infection control protocols to ensure patient safety
Experience:
- Minimum of 1 year of experience as a registered nurse
- Experience in maternal (pre/post natal), Infant health care, family planning is preferred
- Proficiency in using electronic medical record systems is a plus
- Strong communication and interpersonal skills
- Ability to work independently and make sound clinical judgments
- Valid nursing license in Michigan
- CPR certification
