Job description: Program Coordinator
Compassionate Home Health Residential Services Inc. is now hiring a Program Coordinator to provide oversight supervision to individuals with intellectual and developmental disabilities and their direct support staff who work in home like settings. The right person for this position will ensure that our employees are helping the people we support to learn independent living skills, plan activities in the home and community, and implement person-centered plans.
In addition to coordinating and advocating for the people living in the home, other responsibilities include:
- Unit-specific training to new DSPs regarding day-to-day household operations
- Promote the ongoing competency-based training and development of DSPs
- Oversee medication management for all assigned clients; assure adequate supply of medications; review all house medication paperwork (Daily med log, PMOF, MAR, Weekly status report) to assure accuracy
- Understand role and responsibilities of DSP, may work if a DSP is unavailable in an emergency.
- Enforce and abide by all agency and program rules, policies, and procedures
Department Administration:
- Implement and monitor department/agency policies and procedures in residence
- Coordinate with other departments and programs (e.g., individual advocate, nurse, transportation, employment services, admissions, etc.)
- Guide the development and implementation of programming and person-centered activities for individuals
Individual Service Delivery:
- Maintain thorough understanding of person-centered plans and individualized services
- Actively observe for signs of abuse, neglect, maltreatment, or violation of rights and take immediate action to remedy the situation
- Participate in on-call system for after-hours care and support of all program individuals in caseload.
Communication and Documentation:
- Complete and notify the supervisor of incident reports within 24 hours of occurrence
- Maintain communications with the individual's support team, document contacts as appropriate
- Make recommendations for individuals to be referred to different service areas based on performance, preferences, attendance, quality of work and needs
- Set up and maintain house files by program procedures. Complete ABC Data Sheets and Data Collections Sheets for individuals with an approved Behavior Management Plan
Facility Management:
- Residential facilities are the individual's home and should be individualized (decorated/painted/furniture, etc.) of their choice.
- Complete repair request and any household repairs needed to property manager within 48 hours
- Teach and prepare individuals on what to do in emergencies
- Conducted pre-inspections of residence, submitted pre-inspection checklist, and participated in DDA and internal inspections as needed.
Physical Requirements:
- Provide services at multiple locations throughout the community
- Use physical restraints only when such intervention is part of an approved Behavior Management Plan
- Able to complete household cleaning (operate vacuum/mop/wash dishes/ dust, etc.) as needed.
Professional Work Habits:
- Actively participate in intra-and inter-departmental team meetings
- Develop and maintain mechanisms and provide support for agency and departmental cohesiveness
- Facilitate teamwork and positive interactions among teams and between DSP's working within the house.
Job Type: Full-time
Pay: $50,233.00 - $55,632.00 per year
