Partnerships for People, Inc. is currently seeking a Health Services Consultant to join our team of professionals in making a positive difference in the lives of our neighbors and the community.
Responsibilities:
- Provides consultation services for employees and persons receiving services in accordance with identified health care needs and support plans.
- Participates in pre-admission referrals, admissions, discharge/transfer planning for individuals in services as related to health care needs.
- Delivers training and education on relevant medical care and health related topics to employees and individual in services
- Remains abreast of services, topics and best practices in the areas of health care delivery.
- Actively participates in leadership, departmental and other meetings as required.
- Monitors health status of individuals, provides appropriate interventions, and ensures proper documentation.
- Provides on-call services when requested/required.
- Remain abreast of best practices related to health care service delivery for people with intellectual and developmental disabilities
- Adheres to all licensing, federal, state and local regulations as well as health and safety policies.
- Maintains confidentiality standards in accordance with all company and regulatory standards including, HIPAA.

Requirements:
- Must possess current and valid RN or LPN licensure in state will be providing services.
- Minimum of three (3) years of work experience in the medical field, preferably working with individuals with intellectual or developmental disabilities.
- Must possess clinical competencies in all aspects of nursing and ability to apply nursing knowledge and sound judgement.
- Ability to communicate both orally and in writing and to be able to read, interpret and apply written information.
- Computer proficiency in MS Office and able to learn other software as needed.
- Ability to pass required background checks and clearances.
- Valid driver’s license and daily access to privately maintained and insured vehicle.
