The Assessment Specialist assesses the needs of consumers by rating them in the six core areas of their life by completing the Adult Needs and Strength Assessment (ANSA) / Child and Adolescent Needs and Strength (CANS). The Assessment Specialist completes on-going and as needed client mental health assessments, documenting the services provided, progress or treatment barriers identified on behalf of the client at the clinic, in the client's home or community. The Assessment Specialist functions as a central point of communications and customer service about client progress with other members of the La Familia Del Paso client-centered team.
Qualifications:
- Bachelor's Degree from an accredited college or university with a major in social, behavioral or human services related field (Required) (Psychology, Social Work, Medicine, Nursing, Rehabilitation, Counseling, Sociology, Gerontology, Special Education, Physician Assistant, Educational Psychology, Human Growth and Development, Early Childhood Education, Early Childhood Intervention)
- Four (4) years of direct care experience and assessing consumer mental health in community setting with knowledge of rehabilitative techniques and community resources for chronically mental ill (preferred)
- Possess and must maintain Texas ANSA and CANS certification (yearly certification)
- Bilingual (English/Spanish) preferred but not required
- Valid automobile insurance
Essential Duties and Responsibilities:
- Performs Adult Needs and Strengths Assessments (ANSA) and Child & Adolescent Needs & Strengths (CANS) for new consumers and regular consumers as needed
- Have the ability to perform assessments at various La Familia Del Paso Locations
- Determines the level of care that best fits the consumer’s needs based on the ANSA or CANS
- Implements safety plans when suicidal ideation detected in an assessment
- Reports when homicidal ideation detected in assessment to Director of Adult Rehabilitation Services and proper authorities when needed
- Spirit of service and empathy for the mental health population. Refers consumers to other services provided by La Familia Del Paso or other outside organizations when La Familia Del Paso does not provide that specific service.
- Maintains flexibility to perform assessments in a timely manner.
- Participates in compliance functions per federal/state/corporate mandates, regulation, policies, and procedures
- Maintains open communication with supervisor
- Obtains required training hours on an annual basis
- Attends staff meetings and required training sessions
- Will have to travel from site to site to complete assessment as needed
