Congregate Housing Program (In-House) 1 Evening + Weekend Day

Background
Since 1991, Unique People Services (UPS) has grown and now operates transitional, congregate setting, and supportive housing programs in the Bronx and Manhattan. We provide services that include case management that focus on achieving the highest possible level of independent living. We offer transitional, permanent, and long-term housing for formerly homeless persons with substance use, HIV/AIDS, and/or with a mental illness.
Position Summary
As a member of the interdisciplinary team, you will assist in preventing hospitalization, maximize independent functioning ability, assist in avoiding homelessness of mentally ill and substance usage by utilizing engagement skills, motivational interviewing, individual and group counseling, person-centered care, and other therapeutic techniques.
Essential Duties and Responsibilities include the following, but not all-inclusive of other duties that may be assigned
- Provide appropriate Activities of Daily Living (ADL) services in accordance with OMH or DHS recommendations
- Depending on the program the Case Manager will conduct monthly client home visits or as needed
- Attend all mandatory agency and program training
- Report apartment maintenance deficiencies to supervisor in a timely manner
- Liaison with building management/superintendent in order to address clients’ apartment issues
- Collaborate with the interdisciplinary treatment team to maximize coordination of care on clients’ behalf
- Work closely with the interdisciplinary team to cultivate relationships with service providers to assist clients with face-to-face and telephone assistance to link to appropriate services
- Maintain appropriate documents, records, statistics, and reports in an organized, accurate, and timely manner
- Accompany clients to appointments, i.e. medical, social service meetings, hospital emergencies, field trips as necessary
- Facilitate and/or participate in various structured and therapeutic group activities, i.e. vocational skill-building groups, money management
- Depending on the program the Case Manager will process the intake of new clients into Housing Program
Education
- Bachelors Degree in Social Work, Psychology, or other related field of study preferred
Skills and Knowledge (Preferred)
- Minimum of one (1) year experience servicing target populations: homeless, substance usage, HIV/AIDS, LGBTQ individuals
- Excellent written, verbal communication, and presentation skills
- Ability to establish priorities and meet deadlines
- Comfortable working with a diverse population (ethnicity, sexual orientation, socioeconomic status, etc.) of our multicultural community
- Bilingual (Spanish Speaking)
PREFERRED QUALIFICATIONS:
- Community outreach and human services experience
- Knowledge of DOHMH/DHS/HRA/social services agencies as well as NYC community-based supportive services
- Experience with advocacy, client assessments, patient referrals, AWARDS Foothold system

Pay: $40,000.00 - $45,000.00 per year
