We provide a Patient-Centered Medical Home for regular preventive health screening, patient and parent education, and timely and appropriate management of illnesses and concerns.

Job Summary:
The Population Health Coordinator at Cornerstone Pediatrics plays a crucial role in improving the overall health outcomes of the patient population. This position focuses specifically on the unique needs of children and adolescents, aiming to enhance their health and well-being through coordinated care and preventive measures.
Key Responsibilities:
Data Analysis and Reporting:
- Collect and analyze health data to identify trends and areas for improvement.
- Develop reports and dashboards to track health outcomes and quality measures.
Care Coordination:
- Ensure that patients receive timely and appropriate care.
- Coordinate between different healthcare providers and services to streamline patient care.
Patient Outreach and Education:
- Conduct outreach to patients for preventive care, follow-ups, and chronic disease management.
- Educate patients and families on health maintenance, disease prevention, and healthy lifestyle choices.
Program Development and Implementation:
- Develop and implement health programs and initiatives to address specific population health needs.
- Monitor and evaluate the effectiveness of these programs.
Quality Improvement:
- Identify areas for quality improvement within the practice.
- Implement strategies to enhance care quality and patient outcomes.
Community Collaboration:
- Work with community organizations and resources to support patient health.
- Build partnerships to address social determinants of health.
Qualifications:
- Previous experience in a healthcare setting, preferably in pediatrics.
- Strong analytical skills and experience with data analysis and reporting.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with healthcare providers, patients, and community organizations.
- Knowledge of healthcare regulations and standards.
- Commitment to improving health outcomes for children and adolescents.

Pay: $17.00 - $20.00 per hour
