Case Manager - FAST Program
Status: Full time – 32-40 hours per week
Reports to: Director of FAST
Location: 434 Jamaicaway, Jamaica Plain, MA 02130
Job Summary: Join us in our mission to empower unaccompanied, homeless youth through our Freedom to Achieve Stable Tenancy (FAST) program. The FAST Case Manager will oversee housing search activities, providing comprehensive support to help youth obtain housing placement and ensure long-term tenancy.
Key Responsibilities
- Support Services: Education and assistance with housing search and applications, opportunities through various housing authorities, agencies, and companies.
- Housing Search: Ongoing housing search support, including landlord and management company contact, client accompaniment to viewings, and housing agency appointments.
- Assessment: In-depth service needs assessment to determine barriers to stabilization and sustained housing.
- Resource Provision: Credit repair, move-in assistance, CORI/adjudication support.
- Community Development and Partnerships: Develop partnerships with external providers.
- Data Tracking and Reporting: Implement housing intake, tracking, planning, and follow-up engagement strategies for tenancy preservation.
- Staff Development: Participate in professional development opportunities.
- Supervision: Attend case conference meetings; maintain effective communication with staff and agency administration.
- Teamwork: Work as part of a team with the ability to work independently, demonstrating excellent time management and organizational skills.
- Professional Conduct: Adhere to the expectations and policies of ESAC, including respect, cultural sensitivity, organization, and punctuality.
Qualifications
- Required: 2-3 years of case management experience with homeless individuals and families, navigation of legal, financial, and behavioral health services for housing stabilization, knowledge of service agencies and resources for homeless youth and young adults. Sensitivity and understanding of diverse populations. Ability to travel and meet with clients in their home communities.
- Preferred: Candidates with lived experience and reflective of the community served by EYOC.

ESAC has 15 staff who operate the following programs: Senior Home Repair/Falls Prevention, GED Plus, Mortgage Assistance, Benefits Enrollment, Housing Stabilization. ESAC serves 1,200 clients per year in Boston and surrounding communities.

Job Type: Full-time
Pay: $52,000.00 per year
