Duties:
- Provide compassionate and holistic care to patients in hospice settings
- Conduct physical examinations and assessments of patients
- Administer medications and treatments as prescribed
- Monitor and manage symptoms and pain levels
- Collaborate with interdisciplinary team members to develop and implement care plans
- Educate patients and their families about the disease process and end-of-life care options
- Assist with emotional support and counseling for patients and families
- Coordinate with other healthcare providers to ensure continuity of care
- Document patient care activities accurately and timely
Skills:
- Proficient in medical terminology and documentation
- Knowledge of primary care principles and practices
- Familiarity with home care services and community resources
- Competent in phlebotomy techniques for blood collection
- Skilled in IV infusion therapy
Requirements:
- Valid nursing license in Texas
- Minimum 1 years of experience in hospice or palliative care
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
- Compassionate and empathetic nature
