Job Title/Position: Nurse Practitioner
Reports To: lead NP and hospice director
JOB DESCRIPTION SUMMARY
The registered nurse practitioner provides holistic, hospice and palliative patient care within scope of practice and via integration of basic palliative care interventions with their population-focused practice.
Population served: adults and geriatrics
Setting: on the field at patient's private homes, care homes, living facilities, skilled nursing facilities
Location: Santa Clara and San Mateo counties
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Provide timely and effective patient care with emphasis on coordination with RNs, LVNs. This includes in-person visits, telephonic calls with team members, as well as HIPAA compliant messaging system. Frequency and duration of visits are accordant to acuity of symptoms and nursing triage reports.
2. Ongoing symptom management for empaneled patients including supervising assigned RNs and LVNs and provide appropriate pharmacological and non-pharmacological interventions/ orders
3. Apply nursing processes and nursing care as indicated
4. Timely evaluation and approval of controlled substances prescriptions as indicated
5. Provide holistic care approach that aligns with the concept of total pain for end-of-life including utilizing and referring to other interdisciplinary team providers such as MSW, chaplain, volunteer coordinator.
6. Consult hospice MD/DO as indicated
7. Attend interdisciplinary group meetings according to group schedule
8. Oversee empaneled patients to conduct timely face-to-face visit and present clinical findings during IDG meetings
9. Assist in reviewing hospice qualifications of referrals as requested by intake, marketing, community liaison, hospice medical director, DPCS.
10. Attend mandatory monthly provider meeting.
POSITION REQUIREMENTS:
1. Hold active license in state of CA: RN, NP including furnishing license, CPR/AED, NP certifications (AANP or ANCC)
2. Must have active DEA and NPI (will assist with applications if needed)
3. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
4. Experience in electrical medical/health record including charting and reviewing
OPTIONALS:
1. Experience in home-health, field-based medical practice (mobile clinics, mobile providers, outreach, etc.)
2. Advanced certified hospice and palliative nurse (ACHPN) and/or certified hospice and palliative nurse (CHPN) or willingness to pursue certifications
Job Types: Full-time, Part-time
Pay: $180,000.00 - $190,000.00 per year
