Premieant is looking for a full-time Registered Nurse (RN) in the San Antonio area to join our team! A RN combines case management and home health to monitor the health needs of the individuals and intervene when necessary and is responsible for coordinating care and delivering nursing services in accordance with the Board of Nursing (BON) and Department of Aging and Disabilities Services (DADS).

We are a privately owned and operated company that delivers services to Individuals with intellectual and developmental disabilities through ICF-ID, HCS, and Day Habilitation programs. The ICF-ID program serves Individuals in a group home setting, while the HCS program serves Individuals in a variety of residential settings, semi-independent or independent settings. The Day Habilitation program, provides training in a day program setting. Both ICF-ID and HCS programs are funded through Medicaid and are licensed and/or certified annually by the Texas Department of Aging and Disability Services (DADS).
Qualifications/Requirements:
- Associate degree is required, Bachelor degree is preferred.
- One year of general nursing experience, experience preferred in the Intellectual and Developmental Disabilities (IDD) field.
- License: Registered Nurse in Texas by the Board of Nursing; current and in good standing.
- Lift/transfer minimum of 50 pounds, bend, kneel, stoop, reach/ reach overhead, walk/stand for extended periods, sit for extended periods, finger/hand/wrist dexterity to eye.
- Texas driver license with eligible to drive status.
Job Description:
- Perform health care procedures/orders prescribed by physician/medical practitioner and required by standards of professional practice or law to be performed by licensed nursing person including but not limited to administering medicine if the individual is unable to self-medicate
- Perform RN comprehensive assessments and reviewing quarterly and focused nursing exam on Individuals;
- Prepare and follow-up on consumers medical care plan, as changes are needed;
- Alternate on call with other RNs
- Verify the experience and competency of the unlicensed person to perform the task, including the unlicensed person’s ability to recognize and inform the RN of consumer changes related to the task.
- Instruct the unlicensed person on the delegated task and develop all inservices for unlicensed personnel
- Conduct self-medication administration, infection control, and signs and symptoms of illness in orientation of new employees as needed
- In-service training with staff and consumers under RN delegation
- Medical records audit and review of MARS, Physician Orders, Annual Nursing Assessment, labs
- Audit of medical supplies, working in coordination with QIDPs, Program Directors, Assistant Program Directors, house managers, physicians, and pharmacist
- Member of the Consumer Care Committee and attend and participate in quarterly Committee meeting
