SUMMARY OF RESPONSIBILITIES:
· Provide a comprehensive assessment and develop a plan of care in order to address the patient and family psychosocial needs.
· Implement, coordinate, evaluate program services and advise families about available resources and information.
· Prepare written assessments, progress notes, reports, letters, and other communications with patients, other medical providers, and other involved parties.
· Collaborate with families and interdisciplinary team to communicate information, resolve issues and provide services in compliance with established guidelines.
· Serves a liaison between social work department and management team.
ESSENTIAL FUNCTIONS:
· Complete a comprehensive assessment and develop a plan of care to address the patient and family psychosocial needs.
· Provide direct patient care within appropriate licensed discipline, as determined by department need.
· Assist patients and families with community referrals, assist families with completing and submitting applications for insurance programs, handicap placards, lodging and transportation, and other needs as they arise.
· Participate in professional development training activities as instructor and/or attendee related to social work and human resources in the field of rehabilitation.
· Provide information for external services to patients who do not quality for CRIT services.
· Serve as a liaison for Texas Medicaid’s Medical Transportation Program to verify status of eligible patients.
· Consistently and effectively, communicate the social, psychological, and emotional needs of the patient/family to the patient’s rehabilitation team.
· Consult immediately with team when identifying problematic, unsafe, or significant change in patient/family functioning or status.
· Serve as the chair of the external needs committee, which locates medically necessary equipment for patients who are underinsured or uninsured.
· Social worker will oversee the CRIT pantry. Track and report according to San Antonio Food Bank requirements.
· Document social work-related follow-ups, paper work, phone calls and meetings with various entities in EMR.
· Send and receive faxes, scanning in records into appropriate section of EMR.
· Conduct home visits per medical and therapy team’s request.
· Attend ARD meetings as determined by needs of the family and SW.
· Ensures the protection of confidential information in accordance to HIPPA guidelines.
· Coordinate informational sessions or other services for families within interdisciplinary team and external Entities.
· Identify department needs; provide input and oversight on development of programs to be implemented.
· Responsible for oversight of social work department programming and services.
· Other job responsibilities, tasks, and duties may be as assigned by management as needed.
POSITION REQUIREMENTS:
A. KNOWLEDGE/SKILLS/EXPERIENCE REQUIRED:
1. Minimum of 2 years of Social Work experience in a health care setting. Working with children and adolescents preferred.
2. Ability to work autonomously and make independent judgments.
3. Must have a good working knowledge of the impact of illness on the patient and family as well as an understanding of the age specific needs and cultural differences.
4. Must identify and plan for resolution of difficult psychosocial illness related issues and problems.
5. Maintain professional and congenial relationships with patients, patients’ families, community providers, supervisors, and fellow team members.
6. Keep up to date with changes in legislation pertaining to state regulations on social work practices.
7. Teamwork and ability to work in a multidisciplinary team.
8. Excellent communication and educational methodology.
9. Highly motivated in working with patients and families.
10. Excellent writing and documenting skills.
B. QUALIFICATIONS:
EDUCATION/CERTIFICATIONS:
1. Master’s Degree or higher is required of an accredited school of social work.
2. Must hold a Licensed Clinical Social Worker (LCSW) license in the State of Texas.
3. Must possess and maintain current Texas State License.
4. Current CPR certification for Health Care Providers.

Pay: $56,287.00 - $61,014.00 per year
